Section 1.4: LLM Training Pipeline
A raw transformer that has been trained solely on next-token prediction is not useful as a product. It will complete any text pattern it has seen — including harmful, biased, or nonsensical patterns — with no preference for being helpful, truthful, or safe. Turning a base model into an assistant requires a multi-stage training pipeline, and understanding each stage is critical because each stage introduces different capabilities, limitations, and failure modes that guardrails must address.
Overview of Training Stages
The modern LLM training pipeline has three major stages, with additional optional stages becoming increasingly common:

Each stage builds on the previous one. You cannot skip stages — instruction tuning without pre-training produces nothing useful, and alignment without instruction tuning has nothing to align.
Stage 1: Pre-Training
Pre-training is where the model acquires the bulk of its knowledge and capabilities. This stage is astronomically expensive (millions of dollars in compute) and uses massive datasets of text crawled from the internet, books, code repositories, and other sources.
The objective is simple: predict the next token. The model sees trillions of tokens of text and adjusts its weights to become better at predicting what comes next. Through this process, the model implicitly learns:
- Grammar, syntax, and language structure
- World knowledge (facts, relationships, concepts)
- Reasoning patterns (if A then B, mathematical operations)
- Multiple languages and translation patterns
- Code structure and programming patterns
- Writing styles, tones, and formats
- And everything else present in the training data — including toxicity, bias, misinformation, and harmful instructions
Pre-training dataset (simplified):
Wikipedia articles → factual knowledge, neutral tone
Books and literature → narrative, reasoning, diverse perspectives
Web crawl (Common Crawl)→ everything (including toxic content)

What the developer cannot control: The pre-training dataset and process are entirely in the hands of the model provider. By the time you access a model through an API, pre-training is complete. The biases, factual errors, and harmful capabilities baked in during pre-training are permanently encoded in the weights.
What can go wrong: The model learns patterns from whatever is in the training data. If the training data over-represents certain viewpoints, contains systematic factual errors, or includes harmful content (which web crawl data inevitably does), those patterns are encoded with the same fidelity as correct and benign patterns. The model has no mechanism to distinguish “this text appeared in a training document warning about phishing attacks” from “this text is a phishing attack template.”
Why this matters for guardrails: Pre-training creates a model that can generate virtually anything that appeared in its training data. It can write malware because it has seen code. It can generate hate speech because it has seen hate speech. It can fabricate convincing misinformation because it has seen both facts and fabrications in the same statistical space. No amount of post-training can fully remove these capabilities — they are distributed across the weights (as we discussed in Section 1.1). Guardrails exist because pre-training necessarily creates a powerful and potentially dangerous system.
Stage 2: Instruction Tuning (Supervised Fine-Tuning)
After pre-training, the base model is a powerful text completer but a terrible assistant. Ask it “What is the capital of France?” and it might complete it as a quiz question rather than answering it. Instruction tuning bridges this gap.
Supervised Fine-Tuning (SFT) trains the model on curated examples of the desired input-output behavior:

Through thousands of these examples, the model learns to:
- Follow the chat template format (system/user/assistant roles)
- Respond helpfully to questions rather than completing them as text
- Follow instructions, including complex multi-step instructions
- Maintain a consistent persona
- Refuse obviously harmful requests
Chat templates and the instruction hierarchy. SFT introduces the concept of different roles (system, user, assistant) through special tokens in the training data. The model learns a preference for treating system-level instructions as higher authority than user-level instructions. This is the instruction hierarchy — and it is crucial to understand that it is a learned statistical preference, not an enforced constraint.

The model has been trained to weight system instructions more heavily than user instructions, but this weighting is statistical. A sufficiently clever prompt injection can shift the balance. Think of it as a preference with a certain strength, not a firewall.
Why this matters for guardrails: The instruction hierarchy being a learned preference rather than an enforced constraint is one of the most important concepts in guardrail engineering. It means system prompts are not secure boundaries. Anything in the system prompt — safety rules, persona constraints, confidential information — can potentially be overridden or extracted by a determined user. This is why guardrails must be implemented as external enforcement mechanisms, not just model instructions.
Stage 3: RLHF — Reinforcement Learning from Human Feedback
Instruction tuning teaches the model what to say, but RLHF teaches it how to say it in a way humans prefer. RLHF is the primary technique used to align model behavior with human values and safety requirements.
The RLHF process has three sub-stages:
1. Collect comparison data. Human annotators are shown a prompt and two or more model responses. They rank the responses by preference, considering helpfulness, harmlessness, and honesty.
2. Train a reward model. A separate neural network is trained to predict human preference scores. Given a prompt and a response, the reward model outputs a scalar score representing how much a human would prefer this response. This reward model encodes the collective preferences of the human annotators.
3. Optimize with reinforcement learning. The LLM is fine-tuned using Proximal Policy Optimization (PPO) or similar algorithms to generate responses that maximize the reward model’s score, while staying close to the SFT model (to prevent the model from “hacking” the reward model by generating degenerate high-reward text).

RLHF is responsible for many of the safety behaviors we observe in production models: refusing to provide instructions for illegal activities, declining to generate hate speech, acknowledging uncertainty instead of fabricating answers, and following the spirit (not just the letter) of instructions.
But RLHF has significant limitations:
Superficial safety. RLHF optimizes for what looks safe to human annotators, which is not always what is safe. The model learns to produce responses that pattern-match to safe-looking outputs, which is why certain phrasings can bypass safety training — they trigger different patterns than the ones annotators flagged.
Sycophancy. RLHF can inadvertently train models to agree with users rather than correct them, because human annotators preferred agreeable responses during training.
Over-refusal. Models can learn to refuse benign requests that superficially resemble harmful ones, because the cost of a false refusal was lower than the cost of a harmful response during annotation.
Fragility. RLHF-trained safety behaviors can be circumvented through clever prompting, multi-turn manipulation, or by exploiting the gap between what annotators evaluated and what users actually try.
Alternatives to RLHF
Several alternatives and extensions to RLHF have been developed:
Constitutional AI (CAI) replaces human preference labels with a set of principles (a “constitution”). The model critiques its own outputs against these principles and revises them. The revised outputs are then used for preference training. This is more scalable than human annotation but depends on the quality of the constitution.
Direct Preference Optimization (DPO) simplifies the RLHF pipeline by removing the reward model entirely. Instead of training a separate reward model and then using RL, DPO directly optimizes the LLM on preference pairs using a clever reformulation of the RLHF objective. This is simpler and more stable but may be less powerful for complex preference structures.
Reinforcement Learning for Reasoning. Models like OpenAI’s o-series and DeepSeek-R1 use reinforcement learning not for safety alignment but for improving reasoning capabilities. The model is rewarded for producing correct final answers, and through RL it learns to generate extended chains of thought. This creates models that are more capable but not necessarily more safe — in fact, the improved reasoning can be applied to circumventing safety measures.

Distillation
Distillation is the process of training a smaller “student” model to mimic the outputs of a larger “teacher” model. The student learns not just from ground-truth labels but from the teacher’s full probability distribution, which contains richer information about relationships between tokens.

Distillation is relevant to guardrail engineering because safety behaviors do not always transfer cleanly from teacher to student. A large model’s nuanced understanding of when to refuse versus when to help may be simplified in the distilled model. Research has shown that distilled models can lose safety alignment faster than they lose core capabilities, creating models that are capable but inadequately guarded.
Why this matters for guardrails: The training pipeline creates a hierarchy of defenses — pre-training capabilities constrained by SFT instruction following constrained by RLHF alignment. But each layer is probabilistic and imperfect. A guardrail engineer must understand that model-level safety is necessary but insufficient. The model will usually refuse harmful requests because of RLHF. It will usually follow the system prompt because of SFT. But “usually” is not “always,” and the gap between “usually” and “always” is exactly where guardrails operate. Your job is to make the system safe even when the model’s internal safety training fails.
What the Developer Can vs. Cannot Control
Understanding the boundary between model-level and application-level control is essential for making sound guardrail decisions:
The developer CANNOT control:
- Pre-training data and resulting base capabilities
- Provider-level alignment training (RLHF/DPO decisions)
- Internal model architecture and weight values
- Which safety behaviors were trained and how robust they are
- How the model interprets and prioritizes instructions internally
The developer CAN control:
- System prompt content and instruction design
- Input pre-processing and validation (input guardrails)
- Output post-processing and filtering (output guardrails)
- Application architecture (RAG, tool calling, routing decisions)
- Sampling parameters (temperature, top-p, max tokens)
- Choice of model (selecting the right model for the risk profile)
- Monitoring, logging, and alerting infrastructure
- Human-in-the-loop escalation policies
This boundary defines the scope of guardrail engineering. You work with what you can control — the application layer — and you build defenses that compensate for the uncertainty of what you cannot control — the model layer.
Why this matters for guardrails: Every guardrail decision should be evaluated against this boundary. “We’ll add a system prompt instruction to prevent X” is a model-level strategy — probabilistic and bypassable. “We’ll add an output classifier that detects and blocks X before it reaches the user” is an application-level strategy — deterministic and enforceable. The most robust guardrail systems layer both, using model-level instructions to reduce the frequency of harmful outputs and application-level systems to catch what gets through.
